提交联系表时仅获取“消息”部分

Question

使用简单的联系表格。我将它托管在我的服务器上。当我按下“发送”按钮时填写所有信息后，我只会将我在“消息”部分输入的内容发送到我的电子邮件。我该怎么做才能获取其余字段（姓名、电子邮件、电话）。似乎无法确定问题所在。
